She knows it's hard to believe because a lot of you may be thinking severe dysphoria is the worst type of dysphoria. When it comes to pain and the duration of pain and the intensity of pain (whether psychological pain, spiritual pain, physical pain), yes, undoubtedly severe dysphoria is going to be worse. What Dr. Z means by moderate dysphoria being worse than severe is in the sense of how it actually affects you long term. Severe dysphoria is similar to having a very strong migraine—you feel it, you know it's there, and you have to do something to address it. In some respects, having severe dysphoria is in a way better in that it enables one to actually address it because the pain is so acute.
