This is very important because she sees a lot of people going through the surgery expecting completely radicalized drastic results to their face, then getting their hopes absolutely crashed when they don't really get them. That happens for a lot of reasons, but primarily it happens because facial feminization surgery is really not intended to completely give you a fully formed female face—it's just impossible to do that.
Facial feminization surgery is really there to take the features you already have and then do the best it can (depending also on surgeon and their skillset) to feminize those features as much as possibly they can. But sometimes Dr. Z hears people have some crazy expectations like "I'm gonna go under knife and when I come out I'm gonna look completely drastically different to the point where my passability will be absolutely 500 percent and nobody will ever blink an eye."
That doesn't really happen—it happens for some (she'll tell you which categories people fall into), but for a lot of people it doesn't happen.
